Description

The State’s Department of Health Care Services is innovating and transforming the Medi-Cal delivery system through the implementation of California Advancing and Innovating Medi-Cal (CalAIM). CalAIM is a population health approach that prioritizes prevention and whole-person care. CalAIM is designed to offer Medi-Cal members coordinated and equitable access to services that address their physical, behavioral, developmental, dental, and long-term care needs throughout their lives.

The County of San Diego established San Diego Advancing and Innovating Medi-Cal (SDAIM) to assist in coordinating the goals of CalAIM at the local level. SDAIM partners with community groups, direct service providers, Medi-Cal Managed Care Plans, County departments and local governments to provide essential infrastructure and planning to support CalAIM implementation. SDAIM demonstrates the County’s commitment to a healthier region that fosters coordinated and equitable access to health and social care for Medi-Cal members through a cross-sector collaborative approach.

This session is designed to provide a CalAIM 101, showcase local implementation efforts, and invite Live Well partners to provide input on the SDAIM Roadmap to ensure County implementation efforts are in alignment with community priorities.
