Achieving health equity begins with reviewing community assets and needs, analyzing data, disseminating results, and developing a plan. The Public Health Services (PHS) department has contracted with HARC (Health Assessment and Research for Communities) to conduct a Collaborating Partner Survey and a community asset mapping pilot before September 2024. The evidence-based survey will be distributed to community partners and assess PHS’s capacity to address health equity. The asset mapping looks at assets and needs of a community.
During the interactive session, participants will receive an overview of the survey results and learn about the Community Asset mapping process. Information gathered from breakout session will be utilized to support PHS strategic planning, community assessment, and community health improvement planning efforts.
