This session will focus on enhancing healthcare through interoperability between the San Diego 211 Community Information Exchange (CIE) and the San Diego Health Connect Health Information Exchange (HIE). This collaboration aims to share critical community information and patient consent, enriching 211 data with HIE data and ensuring that providers have comprehensive information to support patients, bridging care gaps, refining services, and promoting health equity. Concurrently, a poverty dashboard series in San Diego County visualizes poverty data, highlighting communities with limited access to resources and greater health disparities. Despite San Diego County's low overall poverty rates, specific communities face disproportionate poverty levels, contributing to negative health outcomes. Understanding these dynamic dashboards and data sources is crucial for effective interventions against homelessness, food insecurity, and healthcare neglect in these communities.
Learning Objective:
1. Share best practices, highlight use cases of 211 San Diego/CIE and HIE interoperability and demonstrate opportunities for improved health equity through the availability of actionable community data
2. To identify the areas of persistent poverty (APP), historically disadvantaged communities (HDC), high poverty census tracts, low food access, communities with disproportionate levels of poverty and economic stability within San Diego County.
3. To demonstrate the utility of the interactive dashboards and display the available sources of data related to poverty.
